Apple's new "Spaceship Campus" headquarters opened earlier this year in Cupertino, CA and it started causing issues almost immediately.

So far around 7 people have been injured by the glass doors and walls around the building - and that's only the people that've reported it because they've actually been hurt. The company's new $5 billion headquarters is made of only glass and metal, but despite it's innovative design and forward thinking the building might be breaking some laws.
There's actually a law in California that requires “employees shall be protected against the hazard of walking through glass by barriers or by conspicuous durable markings." So far Apple hasn't been dinged for any problems by OSHA, but if they are they could face some hefty fines.
